Position Summary
Posting Note: Phoenix Children's is establishing a robust Wound Ostomy program that will require our Wound Ostomy nurses to be certified within one year of employment. They will be working closely with patients, families and providers for the care/treatment of wounds and providing the appropriate education to some complex cases. We are looking for Wound Ostomy nurses to be part of our Wound Care and Prevention team! We are hiring for 2 roles, one position is 7 - 4:30, and the other is 2 PM - 10 PM, and both will be 40 hours/week with varying schedules depending on need. The position does require a minimum of ONE YEAR skin/wound/ostomy experience, and you must receive your WOCN certification within the first year of being in this role. Must have a minimum of 3 years of clinical nursing experience, pediatrics preferred. NO NEW Grads, please.
The Wound Ostomy Nurse uses the nursing process and works in collaboration with the healthcare team, patient and family to promote patient skin health through delivery of professional nursing care appropriate to patients age and condition as documented in the nursing and medical plans of care in accordance with evidence-based nursing and patient care practice standards and hospital goals, values and regulatory standards.
Position Duties

Assessment
1. Working in collaboration with the healthcare team, uses specialized knowledge to complete nursing assessments of patient’s skin health, wound, or ostomy condition.
2. Identifies risk factors for infants, children, and young adults with acute and chronic health conditions.
3. Conducts initial and ongoing assessments in accordance with evidence based standards and per policy. -
Planning
1. Collaborates with the health care team, patient and family, formulates individual family centered nursing plan of care to establish plan skin, wound, and ostomy care.
2. Documents plan of care and response on an ongoing basis, as necessary.
3. Participates in patient care rounds and patient care conferences as appropriate.
4. Formulates culturally sensitive, expected nursing outcomes related to skin, wound, and/or ostomy care. -
Implementation
1. Determines and implements appropriate nursing plan of care or escalates as appropriate.
2. Provides expert hands-on nursing care for patients with or at-risk for skin breakdown or pressure injury or with an ostomy.
3. Contributes to team efforts by accomplishing required activities within established parameters to ensure compliance with all applicable specialty guidelines, standards of care, documentation standards, regulations, and requirements.
4. Integrates specialized knowledge into patient and staff education, coaching, and mentoring as applicable. -
Evaluation
1. Monitors patients for treatment effectiveness and increased risk as condition changes; escalates concerns.
2. Revises and updates the nursing plan of care based on current, evidence-based practice, results/efficacy, and patient/family needs.
3. Collects and analyzes pertinent data.
4. Continually evaluates patient responses to skin, wound, or ostomy care. Provides teaching, and discharge plans for wound and ostomy patients as appropriate. Documents patient progress in appropriate charts.
5. Assists in the evaluation of nursing processes, policies, procedures, protocols, and guidelines, making recommendations as appropriate to improve outcomes related to ostomies, skin health and pressure injury prevention.
6. Participates and collaborates in skin health and pressure injury prevention initiatives. -
Performs miscellaneous job related duties as requested.
