Position Summary
Posting Note: The Patient Care Coordinator will support the behavioral health/psych unit by coordinating new patient admissions, discharge planning, and outpatient follow-up care. This role serves as a key liaison between providers, patients, families, and the care team to ensure smooth patient flow and continuity of care.
The incumbent will work closely with Nurse Practitioners and Psychiatrists. Ideal candidate will have experience in case management and/or patient coordination; experience in a behavioral health or hospital setting.
Key Responsibilities:
Coordinate and facilitate new patient admissions into the psych unit
Assist with discharge coordination and outpatient appointment scheduling
Coordination of meetings with patients and families
Coordinate department staff meetings
Support unit operations including ordering supplies for unit
Collaborate with providers and interdisciplinary teams to ensure continuity of care
Utilize centralized computer-based admission/billing systems
Cross-functional responsibilities bridging HUC and PPC functions
Strong behavioral health coordination and case management support
This position is full time, 40 hours/week, M - F, 8 AM - 5 PM; NO holidays/weekends.
This position supports the Inpatient Psychiatry unit by assisting in the coordination of patient care from admission through discharge, through providing administrative support to the entire department, ensuring efficient and effective provision of information across the continuum of care through collaborating with parents, physicians, mental health therapists, nursing and other health care providers to ensure optimal internal and external customer satisfaction.
Position Duties

Customer Service
1. Assists in education of parents regarding behavioral/emotional techniques and support.
2. Serves as positive role model for patients and families by consistently maintaining positive relationships and appropriate boundaries.
3. Demonstrates resourcefulness in finding solutions to customer needs. -
Utilization Review Process
1. Assists with the oversight of the utilization review process by maintaining documentation of the clinical review process, retrieving faxed or telephone requests, identifying appropriate case managers and providers, and forwarding information to appropriate payor sources.
2. Completes discharge utilization review by providing the discharge clinicals to outpatient providers. -
Throughput and Care Coordination
1. As requested by treatment team, contacts and provides basic information to referral facilities, agencies, or services to facilitate care delivery and efficient discharge/transfer.
2. Assists with patient admission process through collaborating with physicians, mental health therapists, nursing and other members of the care coordination team to determine appropriate and timely admissions.
3. Completes tasks related to discharge to home/community as assigned by the treatment team, including but not limited to arranging for emergent services and submission of discharge paperwork to appropriate agencies for continuation of care.
4. Serves as clerical support to Inpatient Psychiatry Department as needed to provide for timely responses to admission inquiries.
5. Arranges for family conferences as requested by treatment team.
6. Arranges follow up appointments as requested by the treatment team. -

2. Assists the physician, nursing, mental health therapists and other treatment team members in decision-making to ensure complete and appropriate information is shared.

Therapeutic interventions
1. Responds to crisis situations appropriately, effectively, and calmly to ensure safety of patients and others.
2. Provides therapeutic care to patients in their daily routines, including meals, school sessions, social and education groups, and activities of daily living to assist in achievement of established treatment plan outcomes for modification of behavior as indicated.
3. Implements behavior management protocols in a safe manner by setting clearly defined limits and utilizing effective therapeutic interventions. -
