Position Summary
Posting Note: Join our team as a Transition to Practice Specialist (TTPS) and play a critical role in supporting nurses as they transition into independent practice or new specialty areas. In this role, you will provide hands-on clinical mentoring, real-time coaching, and skill validation to build confidence, competence, and strong clinical reasoning. Working closely with nurse educators and clinical leaders, you’ll help ensure safe, high-quality patient care while fostering professional growth and successful transitions for nurses across the organization. This position is ideal for an experienced clinician who is passionate about teaching, mentorship, and developing the next generation of nursing practice.
The Transition to Practice Specialist (TTPS) serves as a dedicated clinical mentor and educator for nurses transitioning into independent practice or new specialty areas. The TTPS provides real-time support, coaching, and skill validation to strengthen clinical reasoning, confidence, and competence. Working collaboratively with educators and leaders, this role promotes safe, high-quality care and fosters ongoing professional growth.
Position Duties

Provides bedside mentorship and coaching to promote the development of clinical reasoning, situational awareness, and decision-making skills, guiding nurses in planning and prioritizing care.

Collaborates with department nursing leaders and unit educator to support staff developmental needs.

Collaborates with other disciplines and departments as a change agent to support quality improvement initiatives and regulatory compliance.

Engages in ongoing department endeavors which support the philosophy and values of Phoenix Children's and nursing and the professional development of staff.

Conducts shift-based assessments of staff and provides feedback using standardized tools and clinical guidelines to reinforce safety protocols, documentation standards, and clinical decision-making.

Documents observations, progress, and recommendations in designated reporting systems.

Performs miscellaneous job related duties as requested.
