Position Summary
Posting Note: Phoenix Children’s is hiring a Nurse Specialty Coordinator to support our growing Cardiology program! As a key member of the cardiac care team, this role offers the opportunity to directly improve the patient and family experience during the transition from inpatient care to home.
Position Overview
Due to continued hospital growth, this is an addition to our Cardiology team. The Nurse Specialty Coordinator will manage care for patients post-cardiac surgery or cardiac procedures, coordinating all aspects of discharge planning and post-hospital needs. The role will be highly collaborative with our Cardiac Lead Nurse Practitioner and team of providers.
Key Responsibilities
Coordinate patient discharges from the Cardiac ICU inpatient unit
Manage medications and prescriptions
Arrange follow-up appointments and outpatient care needs
Serve as the primary point of contact for cardiac post-discharge questions
Provide patient and family education prior to discharge
Participate in morning/afternoon rounds to identify upcoming discharges
Support a team of approximately 9 providers (physicians and NPs)
Role Details
Works in the inpatient CVICU, but classified as a clinic position reporting to the clinic
4/10 schedule — NO weekends or holidays
Reports directly to the Clinic; clinical direction and training led by Lead NP
Required Qualifications
Minimum 3 years of hospital-based Cardiology experience
RN candidates must come with strong clinical knowledge in cardiac patient care
No new grads
This position utilizes advanced critical thinking to develop, implement, and evaluate an individualized plan of care that educates, empowers and optimizes patient outcomes throughout all transitions within the continuum of care. It provides leadership, clinical expertise /integration and functional direction to team members in order to ensure quality and continuity of patient services. Serves as the expert resource within a specialty area and participates in assessing specialty population needs and health outcomes. It facilitates continuity of care using nursing process, interprofessional collaboration, coordination of and access to appropriate health care services and community resources.
Position Duties

Nursing Process/Direct Care
1. Implements nursing process of assessment, planning, analysis, intervention and evaluation, incorporating physical as well as psychosocial and developmental needs of designated patient populations.
2. Focuses on a patient population, individual patient or caregiver's needs.
3. Utilizes multiple means of communication including telephone triage, in-person interaction and the use of current and future technologies.
4. Documents patient encounters and required follow-up.
5. Communicates relevant clinical information to the physicians and team to optimize patient outcomes and goals. -
Management of Patient's Condition
1. Acts as partner, advocate, and advisor, assisting and supporting patient/family in the optimal management of their own health care, respecting their culture and values, individual needs, health goals and treatment preferences.
2. Provides patient/family education that focuses on health promotion, management of health issues, and prevention of complications.
3. Utilizes a variety of motivational strategies to effect positive patient/family outcomes.
4. Delegates teaching components to staff prepared to teach and/or intervene in clinical settings.
5. Prepares patient and family as they transition through the care continuum.
6. Facilitates communication between patient, family and interdisciplinary. -
Operations of Clinic Services
1. Works collaboratively with unit / medical leadership to provide guidance and direction to staff by taking an active role in planning, decision-making, and communication to healthcare team of issues affecting the interdisciplinary team, quality of care, and integration of patient care activities.
2. Provides clinical expertise that promotes coordinated specialty services.
3. Assesses staff education needs and provides staff leaning opportunities relevant to patient / family specialty population.
4. Assists in the investigation of significant incidents or identified risk, and partners with leadership to identify opportunities for improvement.
5. May organize operations of clinic site to assure compliance with necessary service requirements of identified population or specialty program.
6. Utilizes professional, regulatory, and governmental agency standards to guide care of patient population. -
Continuity of Care
1. Facilitates continuity of care using nursing process, interprofessional collaboration, coordination of and access to appropriate health care services and community resources.
2. In collaboration with the multidisciplinary team, refers patients to optimal health services within and outside the organization and across the care continuum.
3. To assure optimal care and coordination, supports patient and family in transitioning across the care continuum: hospital based services, community based services and other health care professionals.
4. Facilitates transition from pediatric to adult care as appropriate. -
Assessing Effectiveness of Services and Monitoring Specialty Population Needs and Outcomes
1. Evaluates effectiveness of specialty care services with attention to target population quality indicators, such as unplanned admissions, patient satisfaction, and health / illness / injury outcomes.
2. Maintains skill and knowledge proficiency/competency within specialty area.
3. Maintains accurate records to support data analysis and reporting.
4. As needed prepares accurate, required reports for granting agencies, specialty national health initiatives, and hospital quality initiatives.
5. Regularly reviews research based literature to improve clinical care outcomes and support quality improvement efforts, as well as compliance with regulatory bodies.
6. Facilities adherence to national standards as appropriate. -

1. Promotes the development of community awareness of disease process as appropriate to the focus of the clinical specialty program.
2. Provides community education.
3. Participates in relevant community events. -
