Position Summary
This position is responsible for supporting the Patient Placement Coordinator with transfer requests, patient placement, and transport for all Phoenix Children's (PC) facilities. This position assists in facilitating patient throughput, focusing on quality and efficiency, and collaborating across multi-disciplinary care.
Position Duties
-
Collect and accurately document any supporting information for transfers, admissions, and transports in the appropriate fields and/or software system, to promote effective communication among all members of the clinical team.
a. Updates patient accounts to reflect the correct admission status based on physicians` orders. Accurately completes all status changes in appropriate system.
b. Works across multiple software platforms, ensuring accuracy and completion in all.
c. Creates pre-admission accounts for all unscheduled incoming patients, including emergency department transfers, fast pass and expected patients, and direct to OR transfer patients.
d. Creates quick registration pre-admission accounts for all unscheduled incoming patients, as directed.
e. Creates and completes patient account transfer updates from multiple Phoenix Children's facilities and outside facilities.
f. Collaboration/Notification of Patient Access to ensure insurance notification to payer is complete.
g. Accurately scans outside documents and uploads them into the correct system in a timely manner. -
Transportation Coordination
a. In support of Patient Placement Coordinators, coordinates inter-facility transport with EMS, utilizing established workflows.
b. Monitors EMS performance, escalating issues to Patient Placement Coordinators and all relevant stakeholders as necessary.
c. Communicates necessary transport information between sending and receiving hospitals, as directed. -
Supports the Patient Placement Coordinators in a variety of Connected Care Center duties as appropriate.
a. Monitors and updates BedTracker system, utilizing software to assign patients to beds per protocols.
b. Performs duties as assigned to facilitate throughput.
c. Participates in department-specific process improvement projects.
d. Monitors and appropriately escalates any delays in throughput to the Connected Care Center Patient Placement Coordinators. -
Maintains effective communication and a positive, professional work relationship with providers, clinical personnel, colleagues, and leadership to promote teamwork, collaboration, and a positive public image for the organization. Courteously communicates via telephone, Microsoft Teams, vocera, and/or emails and answers all questions in a timely manner
a. Responsible for timely response to all phone calls, demonstrating excellent customer service skills, and assisting callers.
b. Monitors all chats and vocera messages to ensure timely response. Alerts the CCC Patient Placement Coordinators to any identified potential issues and/or backlogs timely and appropriately.
c. Uploads documentation to Quantum as needed.
d. Documents activities clearly and accurately to promote effective communication with all members of the clinical team. -
Demonstrates problem-solving and conflict resolution skills, along with maintaining a multi-tasking mentality in an effort to produce consistency throughout each day/night shift.
a. Identifies potential problems and brings them to the attention of the Patient Placement Coordinators to prevent/resolve barriers.
b. Performs duties in a self-directed manner, with input from the Patient Placement Coordinators. -
Attends unit and system meetings and educational programs to maintain competency levels.
