This is a 12-month post-graduate year one (PGY-1) pharmacy residency program, structured to provide training and learning experience in pharmacy practice and education. This position is accredited by the American Society of Health-System Pharmacists (ASHP). The Pharmacist Resident will perform daily patient care activities in the medication management process as a registered pharmacist defined by the Arizona Board of Pharmacy Statutes. This position reports directly to the Residency Program Coordinator and receives direct supervision from the Director, Clinical Pharmacy, Education and Research. The Resident also receives oversight and supervision from the Director, Inpatient Pharmacy Operations.

• Accurately processes and dispenses inpatient and outpatient prescriptions and provides oral and written counseling information to the patient and/or caregiver when necessary.
  
  a. Provides oral and written information to patient and families.
  b. Monitors patients for optimal therapeutic outcomes.
  c. Verifies appropriateness of medication selection for patient`s diagnosis and age specific criteria.
  d. Screens for and verifies allergies, dose limits, and interactions for drugs prior to dispense.
  e. Processes prescriber orders in the identified pharmacy information systems in an effective, efficient, timely, safe, and accurate manner and manages the computerized patient profile.
  f. Maintains appropriate competency and skills with automated dispensing and other related medication system technology used within the department and hospital to provide safe care.
• Acts as liaison between the pharmacy and the medical and nursing staff for issues pertaining to pharmacy services and actively participates in the education of pharmacy students and interns.
  
  a. Serves in a professional capacity on unit-based clinical teams providing comprehensive patient care support for medical and nursing staff.
  b. Suggests evidenced-based and effective therapeutic drug regimens for care.
  c. Oversees technical and support staff to promote safe and efficient daily workflow.
• Effectively communicates problems, unique patient needs, and other necessary information for enhanced continuity of care.
  
  a. Responds to all modes of communication within the department and hospital.
  b. Proactively and effectively solves problems for medication management.
  c. Answers phones promptly and politely demonstrating Phoenix Children’s Values of excellence in clinical care.
• Provides direct oversight and timely and accurate verification of final product for all aseptic parenteral and non- parenteral medication preparation, dispensing, and distribution in compliance with state, federal statues, and hospital procedures.
  
• Successfully completes the annual competency and skills validations with a qualified observer by skills lab and/or examination as required. Participates in professional development programs and ensures ongoing awareness and understanding of most current methodologies, practices, and philosophies as applied to the relevant patient population and research area. Actively pursues self-education and ensures satisfactory completion of all State Board continuing education credit currency.

• Completes requirements of the PGY-1 residency program per the residency program manual with progression shown in learning, development, and skillsets. Examples include residency research project, required medication use evaluation(s), drug class review, etc.
  
• Initiates, processes, and completes all associated medication error incident, adverse drug reaction, and compliance documentation reports and records to meet standards and statues. Prepares various other reports as necessary in defined rotation syllabi.
  
• Reviews and interprets physician orders for completeness and appropriateness with respect to the patient’s clinical status, age, and/or other significant factors affecting the safety and efficacy of the drug therapy prescribed.
  
  a. Verifies and ensures appropriateness of medication selection.
  b. Recommends initiation, intervention, and substitution of drug therapy when necessary.
  c. Documents clinical interventions in the identified data base to meet department metrics.
• Ensures doses are filled and delivered within established departmental guidelines in a timely and safe manner. Evaluates need for all non-formulary medication required. Suggests equally efficacious and cost-effective therapy when appropriate.
  
• Performs miscellaneous job related duties as requested.
