Position Summary
The Clinical Manager for Transplant Services is an Advanced Practice Provider who promotes family-centered care by subscribing to and supporting Phoenix Children’s mission and values. This position manages, evaluates and supervises operational functions and activities of the transplant programs at PCH by guiding and supporting staff. He/she also serves as a liaison with other departments to plan, implement, communicate, evaluate and coordinate patient care operations across the enterprise, facilitating quick resolution of issues to ensure excellence in a family centered care environment. He/she also provides oversight and day-to-day management of all clinical staff in the transplant programs, as well as the PA`s and NP`s within the transplant programs. Additionally, the Clinical Manager for Transplant provides care to a caseload of patients within his/her scope of practice as outlined by applicable state law, regulations, institutional policies and practice agreements. He/she functions as an independent healthcare provider who, working collaboratively within a multidisciplinary health team including transplant surgeons and physicians, is responsible for providing comprehensive care to pediatric transplant patients in both inpatient and outpatient settings. The Clinical Manager for Transplant Services provides care and educates patients and their families in transplant related care. He/she develops criteria for and participates in the evaluation of the quality and effectiveness of care. He/she plans and participates in learning opportunities for advanced practice, nursing, medical students, other healthcare professionals and community members as it relates to the care of the transplant patient. He/she works with administrative, nursing, physician, and ancillary staff to assure quality patient care and to promote education and research.
Position Duties

Clinical Leadership
- A.Maintains 24/7 accountability for patient care operations within all transplant programs. Works with Transplant Medical, Surgical and Hospital leadership to ensure continuous care coverage.
- B.Provides leadership for transplant subspecialty Advanced Practice Providers in the department, including scheduling, payroll, budget, staff evaluations/counseling and recruitment.
- C.Assists with collaborating physicians on professional development of transplant Advanced Practice Providers.
- D.Actively participates in transplant programmatic development.
- E.Partners with transplant surgeons, physicians and hospital leadership to develop and communicate the vision for the physicians/nurse practitioners/physician assistant/clinical nurse specialist/RN/MA care delivery models. Facilitates the implementation and ongoing revisions of models across the patient care continuum including critical care, step-down, medical/surgical, and ambulatory settings.
- F.Responsible for collaborating with colleagues to develop and implement the patient care philosophy, standards of practice, department goals, policies and procedures.
- G.Ensures continual delivery of high quality clinical care.
- H.Assures compliance of The Joint Commission, CMS, OPTN standards as well as other regulatory agencies.
- I.Enforces adherence to relevant state boards for clinical personnel and to other governing agency regulations; measures health outcomes against standards; makes or recommends adjustments.
- J.Remains current and works towards best practice in clinical management, clinical knowledge and PCH policies and procedures. Incorporates clinical expertise by serving as a resource related to clinical aspects of areas of responsibility

Inpatient/Outpatient Clinical Responsibilities
- A.Inpatient Clinical Care Responsibilities
- i.Conducts comprehensive health history and physical assessment of patients with acute health problems.
- ii.In accordance with the written practice agreement, establishes medical diagnoses, orders and evaluates diagnostic studies, and develops treatment plan, which may include inpatient hospitalization. Performs admission history and physical, and writes admission orders.
- iii.Communicates with transplant physicians/surgeons/resident on the team to develop treatment plan.
- iv.Makes rounds to observe and record pertinent progress of inpatient transplant patients. Attends rounds as a liaison of the Transplant team. Notifies the physician of changes in the patients’ condition and adjusts treatment plan accordingly.
- v.Rounds on transplant patients admitted to the hospital.
- vi.Facilitates referrals to appropriate community health facilities, agencies and resources.
- vii.Discharges hospitalized patients at the direction of the supervising physician, discharge coordination in conjunction with social work and case management.
- viii.Prepares and dictates discharge summaries of patients’ hospitalization
- ix.Follows-up on radiology and laboratory studies
- x.Ensures collaborative relationship between the transplant multidisciplinary team members and subspecialty services
- b.Outpatient/Clinic Care Responsibilities
- i.Conducts comprehensive or episodic health history and physical assessment of patients/families with acute, chronic and/or potential health problems.
- ii. Establishes medical diagnoses.
- iii.Accurately and appropriately transcribes or executes standing orders at the direction of the supervising physician. Writes specific orders within the limits of the practice agreement/scope of practice.
- iv.Discusses proposed procedures with attendant risks and potential complications to obtain proper patient informed consent.
- v.Sees patients in follow-up for routine care.
- vi.Provides appropriate direction to clinic staff regarding patient care issues.
- C.General Clinical Responsibilities
- i.As needed, provides developmentally appropriate clinical care within professional domain to ensure delivery of quality integrated developmentally appropriate patient care; participates as appropriate in emergent situations.
- ii.Provides for continuity of care between clinic visits.
- iii.Analyzes clinical practice and identify opportunities to increase effectiveness and efficiency.
- iv.Meets regularly with appropriate physicians, advanced practice providers, and key interdepartmental stakeholders; work collaboratively to effect programmatic changes and develop protocols.
- v.Works in collaboration with attending physicians, residents, fellows, and nursing staff to provide on-going patient care and management.
- vi.Is responsible for timely and accurate written documentation or dictation of patient care encounters/procedures.
- vii.Communicates effectively with referring health care providers.

Human Resource Management
- A.Manages all aspects of personnel administration for transplant clinical programs including by not limited to recruitment, interviewing, selection of professional, technical or supervisory staff.
- B.Assesses necessary Advanced Practice, RN and MA personnel are available to meet fluctuating census, acuity and budgetary goals. Implements innovative and effective recruitment, retention and on boarding strategies in collaboration with HR and other departments/disciplines to meet unit needs as well as to foster growth and staff satisfaction.
- C.Implements strategies to foster staff’s high engagement, maintain low staff turnover, and support high level of staff satisfaction.
- D.Assists staff with goal setting; implements continual performance development.
- E.Evaluates staffing patterns/needs; match staff competency with patient acuity.
- F.Effectively manages grievances in collaboration with appropriate leadership and Human Resources, monitors staff for fitness for duty, performance improvement and initiates corrective action as appropriate.
- G.Works collaboratively with Clinical Supervisors to identify staff performance issues/needs, provides feedback and support to address staff issues requiring immediate intervention, and identifies educational needs to ensure maintenance of established performance, competency, and quality standards.
- H.Develops and supervises staff through provision of timely feedback and use of appropriate Human Resource policies and tools to facilitate and ensure the ability of staff to achieve highest level of professional growth and desired performance standards.
- I.Collaborates with Clinical Education Specialists for orientation and ongoing professional development of all staff.

Financial Management
- A.Recognizes the impact of reimbursement on revenue. Monitors to ensure accuracy of expenses within transplant cost centers.
- B.Provides timely review and feedback to management team for fiscal responsibilities of designated areas reflecting optimal utilization of all resources.
- C.Works in collaboration with other leaders and departments to develop, secure approval for, and monitor revenue, expenses and capital budget for areas of responsibility.
- D.Reviews financial data monthly and recommends/implements strategies to ensure compliance with approved budgets.
- E.Conducts ongoing evaluation of productivity.
- F.In partnership with Transplant Finance ensures compliance with Transplant Time Studies for clinical staff under his/her leadership.

Strategic and Quality Management
- A.Establishes and oversees transplant goals in conjunction with leadership and organizational goals/priorities.
- B. Supports the growth of Phoenix Children’s; participates in the development and implementation of programs, as applicable.
- C.Oversees all aspects of care delivery, both direct and indirect, for areas of responsibility.
- D.Identifies key performance indicators; establishes data collection methodology; evaluates performance data; responds to outcome measurement findings; complies with documentation requirements.
- E.Participates both proactively and retroactively in assessing and reviews quality of care by utilizing modalities such as rounding, data collection through dashboards, etc. and participating in event reviews, as appropriate.
- F.Facilitates, attends and participates, as appropriate, in medical staff, hospital and patient care area committees.
- G.Facilitates a structure of shared governance; support a just culture.
- H.Monitors/reviews safety reports and collaborates with Clinical Supervisors and Transplant Quality Consultants for trends; develops and implements strategies to address drifts in practice.
- I.Assesses customer and patient satisfaction; develop strategies to address satisfaction issues.
- J.Participates in quality assessment and improvement activities. Identifies important aspects of care and assures findings are used to improve practice and operations. Plays an active role in transplant performance improvement program initiatives

Education and Outreach
- A.Participates in appropriate Grand Rounds, patient selection conferences, quality meetings, conferences, staff in-services and patient education classes.
- B.Identifies and establishes clinical learning opportunities for, NP/PA students, residents, Registered Nurses, and other health science students in conjunction with appropriate faculty, monitor students’ performance. May act as preceptor in student learning experience.
- C.Participates in formal programs such as clinic and hospital in-services; community conferences, CME workshops and courses, and informal education activities such as reviewing current literature, independent study and consultations with health professional and experts in related fields. This includes active participation in relevant professional organizations.
- D.Develops care pathways, protocols, and guidelines for management of specific problems in collaboration with physicians and other members of the health care team.
- E.Develops, implements, and evaluates comprehensive patient education programs that assure quality and appropriateness of care across settings Supports the institution`s outreach mission through consultations and/or participation in health promotion, health education and specialty requests from community agencies.
- F.Acts as a resource to health care staff and implements educational programs to improve care provided to patients/families. Identifies areas of learning need and provides information, refers to information sources and /or assists the individuals or groups to increase skills.
- G.Develops state of the art collaborative practice relationships to serves as models for nursing and other students in the health science disciplines.

Research
- A.Evaluates current research within area of expertise/practice for applicability to the care of patients.
- B.Supports the planning of and participates in clinical research conducted by students and faculty of the Institution and /or other members of the health science disciplines.
- C.May perform independent research, acting as principal investigator in research.
- D.Works with transplant data and quality team members to ensure quality documentation and data abstraction for the transplant programs.
- Performs miscellaneous job related duties as requested.
