Position Summary

This position assists patients and families in the medical environment, as well as provides therapeutic interventions with patients and families experiencing mental health issues under the supervision of licensed staff in compliance with the ASU affiliation agreement and student learning contract.

Position Duties

  • Completes biopsychosocial assessments that include documentation including, but not limited to presenting issues, mental health issues, family composition and strengths, cultural strengths, community resource needs, issues of grief and loss, emotional support assistance, psycho-educational needs to enhance medical adherence, barriers to successful disposition from the hospital, safety issues, and issues relating to social determinants of health and communication needs.
  • Formulates treatment plans to address identified mental health issues, including treatment goals, progress in meeting goals, and interventions utilized. Formulates plans of care to assist in ensuring successful discharge outcomes from a psychosocial perspective. This includes compliance with mandated reporter responsibilities and understanding and utilizing the Social Work Code of Ethics as guiding principles in working with patients, families and colleagues.
  • Actively participates in training and education opportunities at PCH to learn evidence based practices and how to develop empathic and effective therapeutic relationships with patients and families.
  • Performs miscellaneous job related duties as requested.
