Position Summary
Posting Note: The Phoenix Children’s is hiring a Lead Child Protection Team RN. The Lead role will be the point person for education, orientation, competency, team schedules and assignments of the SANE RN Team. The Child Protection Team at Phoenix Children’s partners with agencies such as law enforcement, children’s services, victim advocates, and mental health clinicians.
Sexual Assault Nurse Examiners work with a team to provide services at Phoenix Children’s as well as three advocacy sites – Childhelp Children’s Advocacy Center of Arizona, Mesa Family Advocacy Center, and Southwest Family Advocacy Center in Goodyear. The Team consists of three full time pediatricians, four nurse practitioners, five forensic nurses, three dedicated forensic interviews and five medical assistants. This position is located at our partnership with Child's Help, and located at 2120 North Central Avenue, Phoenix, AZ. The hours of this position will be 2 PM - 10 PM, which qualifies for the second shift, shift differential!
The Lead Child Protection Team RN is responsible for being the point person to lead the Sexual Assault Nurse Examiner (SANE) team regarding education, orientation and ongoing competencies as well as team schedules and assignments. Escalates issues pertaining to difficult cases, serve as the designated liaison with the providers and other interdisciplinary team members. This position is 10% administrative and requires a clinical knowledge base to help facilitate patient care, overcoming barriers to care, helping to improve the overall quality of care and enhancing the patient experience.
Position Duties

Nursing Process/Direct Care
- Completes nursing assessment of patient conditions consistent with age and condition, including physical, psychological, social, developmental, and educational factors, to determine initial level of compliance with treatment plan per policy and unit guidelines. Performs ongoing patient and family assessments and identifies variations from diagnoses.

Management of Patient's Condition
- Acts as partner, advocate, and advisor, assisting and supporting patient/family in the optimal management of their own health care, respecting their culture and values, individual needs, health goals and treatment preferences.
- Continually evaluates patient responses to care, teaching, and discharge plans. Documents patient progress in appropriate charts and shares observations and assessment outcomes with healthcare team and families to ensure timely communication and understanding of patient health status, progress, and ongoing needs.
- Delegates teaching components to staff prepared to teach and/or intervene in clinical settings.
- Facilitates communication between patient, family and interdisciplinary.

Operations of Clinic Services
- Works collaboratively with unit / medical leadership to provide guidance and direction to staff by taking an active role in planning, decision-making, and communication to healthcare team of issues affecting the interdisciplinary team, quality of care, and integration of patient care activities.
- Provides clinical expertise that promotes coordinated specialty services.
- Assesses staff education needs and provides staff leaning opportunities relevant to patient / family specialty population.
- Assists in the investigation of significant incidents or identified risk, and partners with leadership to identify opportunities for improvement.
- May organize operations of clinic site to assure compliance with necessary service requirements of identified population or specialty program.
- Utilizes professional, regulatory, and governmental agency standards to guide care of patient population.

Continuity of Care
- Facilitates continuity of care using nursing process, interprofessional collaboration, coordination of and access to appropriate health care services and community resources.
- In collaboration with the multidisciplinary team, refers patients to optimal health services within and outside the organization and across the care continuum.
- To assure optimal care and coordination, supports patient and family in transitioning across the care continuum: hospital based services, community based services and other health care professionals.
- Attends MDT Case Review meetings. Assists law enforcement agencies with required paperwork related to medical evaluations as appropriate. Assesses barriers that prevent children from accessing medical evaluations and working with partner agencies to address them.
- Facilitate, coordinate and lead SANE MDT case and peer review process monthly. Participates in quarterly 1:1 Forensic Nurse Examiner “update” meetings with Medical Director.

Assessing Effectiveness of Services and Monitoring Specialty Population Needs and Outcomes
- Evaluates effectiveness of specialty care services with attention to target population quality indicators, such as unplanned admissions, patient satisfaction, and health / illness / injury outcomes.
- Maintains skill and knowledge proficiency/competency within specialty area.
- Maintains accurate records to support data analysis and reporting.
- As needed prepares accurate, required reports for granting agencies, specialty national health initiatives, and hospital quality initiatives.
- Regularly reviews research-based literature to improve clinical care outcomes and support quality improvement efforts, as well as compliance with regulatory bodies.
- Facilities adherence to national standards as appropriate.
