Position Summary

Posting Note: The incumbent must have the following qualifications: BSN with 3+ years of clinical practice experience in Hematology/Oncology and experience in Bone Marrow Transplant is preferred. This position will work to support patient from referral to transplant through admission for transplant. This includes donor search, patient evaluation work up and coordination with providers within the team. This position requires working closely with outside vendors such as cell labs and the National Marrow Donor Program. Experience in providing patient education will be an important part of this role. This is a M-F Position, 4/10's-no weekends/holidays. Up to $10,000 in incentives and Relocation Assistance available.

The Clinical Transplant Coordinator (CTC) is a Registered Nurse (RN) and a designated member of the transplant multidisciplinary team with primary responsibility to coordinate and ensure continuity of care for transplant candidates, recipients and their families throughout all phases of care (referral, evaluation, waitlist, transplant surgery and post-transplant).The CTC partners with the members of the multidisciplinary team including, but not limited to physicians, surgeons, nurses, social workers, pharmacist, nutritionists, financial coordinators and administrative personnel.

Position Duties

  1. Direct Patient Care
    • a.Delivers direct patient care and supervises nursing care provided by assistive personnel.
    • b.Facilitates and coordinates evaluation of potential transplant candidates. In the case of solid organ transplant also lists patients with OPTN and provides waitlist management.
    • c.Develops, monitors, implements and coordinates comprehensive care plans for the transplant candidate or recipients under protocol including medication management; assessment and treatment of complications; laboratory management; interventions and discharge planning in both the inpatient and outpatient setting.
    • d.In some transplant program’s role may be responsible for participation in continuous call coverage.
  2. Manages patient and program information – including relevant consents.
    • a. Obtains, compiles and manages candidate and recipient records as required by program and regulatory guidelines.
    • b.Enters data into required locations, including transplant specific database, following defined workflow.
    • c.Provides clinical information required to various entities including payors, research and other departments.
    • d.Obtains and maintains consents
    • e.Partners with transplant finance, payors and others inside and outside the organization to obtain required authorizations for care, and to ensure prescribed care is obtained and maintained.
  3. Communication
    • a.Communicates clinical assessments, findings, and plan of care to members of the transplant multidisciplinary care team, the transplant candidate, recipient, and family. This includes verbal and written communication to both internal and external audiences.
    • b.Manages appointments, testing and medications with the multidisciplinary care team, transplant candidate, recipient and family.
    • c.Participates in multidisciplinary transplant meetings, including, but not limited to patient selection committee, daily round and huddles.
  4. Quality, Compliance, Education and Professional Development
    • a.Acts as a communication liaison between patient, family, and multidisciplinary team by providing teaching and support to recognize limitations and strengths within the patient and family system.
    • b.Attends and participates in program specific quality meetings and performance improvement projects.
    • c.Performs data collection and entry as required for the program’s quality and compliance
    • d.Serves as a clinical role model, patient advocate, and resource to staff, patients, and families. Share’s knowledge and skills of specialty area to assist problem solving collaboratively
    • e.Maintains education and current working knowledge of policies and procedures unique to the transplant program.
    • f.Maintains education and current working knowledge of regulatory requirements unique to the transplant program and the role of the transplant coordinator.
    • g.Participates in ongoing continuing education specific to the transplant program.
  5. Performs miscellaneous job related duties as requested.
