Position Summary

The goal of care coordination is to assist in managing care, cost, and outcomes across the continuum of care. The Manager of Population Health supports the Clinically Integrated Networks Disease Management, Utilization Management, Case Management, and Population Health initiatives. This position works collaboratively with Clinical Integration in developing and implementing an electronic health information system. Additionally, this position works with data analysts to provide the care coordination team high level technical and analytic support. This includes, but is not limited to, identifying high cost/high risk members and performance analysis, which will guide the care coordination team.

Position Duties

  1. Works collaboratively with the health information system development team during building and implementation to ensure care coordination technology platform supports the overall organization values and mission.
  2. Educates and provides technical assistance to the care coordination team during the health information system implementation and provides ongoing technical support.
  3. Serves as liaison between data analytic staff, payers and care coordination team to analyze and interpret data for performance and utilization trends, and identify opportunities for improvement.
  4. Understands all PCCN contracts and risk arrangements to organize clinical reporting systems that support effective, efficient care delivery and optimal outcomes.
  5. Works with a strategic leadership team to design and implement programs and initiatives to ensure high quality, cost effective care across all settings
  6. Measures and reports outcome metrics related to accepted clinical practice and/or evidence-based guidelines, quality standards, care management and financial measurements of service delivery efficiencies and associated costs of care.
  7. Provides care coordination team with clear direction. Shares knowledge with team, acts as resource for staff, answers questions, provides workarounds for obstacles that arise.
  8. Performs miscellaneous job related duties as requested.
