Position Summary

The Social Services Tech (SST/Coordinator)- is a part of the Phoenix Children`s Care Network (PCCN) Care Coordination Team. The SST performs a wide range of duties that promote a positive patient-family experience; ensure families have access to necessary resources; and builds collaborative opportunities with local social service agencies, school districts, and mental health providers. The Social Services Tech (SST/Coordinator)- interacts directly with patients and families, community social service agencies, other healthcare organizations, mental health providers, schools, and third-party payers. This position reports to the Manager of Integrated Care Coordination and will interact regularly with the team`s LMSW. This position does not supervise any other position.

Position Duties

  1. Social Work Activities
    • a) Assess and assist families with resources including: information on where to obtain basic necessities such as clothing or food, specialty clinic information such as free and low-cost clinics for adult dental needs, WIC information, housing resources, mental health resources, school accommodation resources, and other social service agency contact information. Provides resource and education information as requested by patients, families, and providers. Will utilize PCH Emily Center for basic, vetted medical resources/education.
    • b)Assists patients-families to schedule needed appointments with providers and community agencies.
    • c)Will perform outbound calls to assist patients in obtaining necessary services, to close gaps in care, and to facilitate calls to providers and payers.
    • d)Faxes referrals, medical records, and other documents, as needed to various community agencies, providers, and other healthcare organizations.
    • e) Assists patients with documents or letters needed for FMLA, school accommodations, and other social needs related to their care. May participate/assist in completion of DDD, Altcs, food stamp applications; may receive guidance from LMSW as needed.
    • f) May participate in meetings with internal and external members of the care team, such as Interdisciplinary Care Team meetings and school accommodation meetings
    • g) Appropriately escalates complex cases or unorthodox requests to LMSW, RN, or ICC manager.
    • h)Appropriately escalates DTO/DTS cases to LMSW or ICC Manager immediately
    • i)Will perform some CN dutuies/tasks in this position:
    • 1) Case load assignments
    • 2) Provide outreach
  2. Team Support
    • a)Works collaboratively with team members and performs cross coverage for Care Navigators and CM Tech
    • b)Assists LMSW with ongoing training/educating team on basic social service/mental health needs
    • c)May assist in training of new staff
    • d)Works with partner SST to keep resources updated on team drive
    • e)Will be subject matter expert on hard to find or rare resources
    • f)Contributes to the development and enhancement of Pediatric Care Constellation (PCC)
  3. Provides excellent customer service
    • a) Responds to emails within 24 hours.
    • b) Communicates clearly in both written and verbal communications with all internal and external customers.
    • c) Provides excellent service routinely in interactions with all internal and external customers.
    • d) Is willing to continue to hone skills for patient and family engagement strategies
  4. Performs miscellaneous job related duties as requested.
