Position Summary

This position leads quality improvement activities, collaborates with physicians, nurses, dietitians, and researchers and acts as the primary resource to the Endocrine department staff responsible for care of the CARE Clinic patient. This position develops and manages program operations including budgets, funding proposals, internal and external customer relations, and supervising CARE Program staff, to ensure the program complies with established objectives and goals. This position works closely with the PCH Medical Staff, Administration, and Professional Community through project planning and process implementation. The CARE Program Manager is responsible for working with the Division Chief, Division Operations Manager, the CARE Medical Director, and attending physicians in managing all aspects of the CARE Program for the Division of Endocrinology and Diabetes.

Position Duties

  1. Program Development
    • Provides leadership for complex program initiatives by assessing conditions, communicating goals and strategies, providing direction and support.
    • Works closely with departmental and hospital administration to assess and develop programmatic and departmental policies and procedures.
    • Liaison with multitude of hospital departments, including various medical specialties, finance, human resources, managed care, patient access, institutional review board, revenue cycle, medical records, marketing and communications, the Foundation, and upper administration to meet programmatic and division needs, as well as ensure compliance with state and federal policies, rules, laws and regulations.
    • Conducts literature review of identified topic(s), collects data; develops and implements programs and projects based on outcomes as needed.
    • Participates in a variety of professional development programs to ensure ongoing awareness and understanding of most current methodologies, practices and philosophies as applied to the relevant patient population and research areas.
    • Produces timely reports to evaluate the program. Prepares succinct and informative project reports for the program, hospital administration, division, and funding sources.
  2. Operations Management
    • Oversees daily operations of the program, which includes but is not limited to managing staff in developing and maintaining a patient care coordination system including referrals, appointments, follow-up testing, education, research, quality improvement assessments, and community programs.
    • Manages and develops program staff, students, and volunteers, which includes, but is not limited to, recruiting, assessing, supervising, supporting, training, and succession planning.
  3. Financial Management
    • Provides financial oversight for program. Prepares and analyzes fiscal budgets and projects revenue and expenses. Oversees purchases and billing. Makes recommendations for adjustments as needed in conjunction with the offices of Finance and Revenue Cycle, to maintain financial sustainability of program. Assists with funding activities, including identifying opportunities, writing grants and funding proposals. Oversees all programs funded. Prepares funding reports as needed for administration and funding organizations
  4. Community Networking
    • Establishes, manages, and evaluates relationships with community organizations, industry, philanthropic organizations, individual donors, and community stakeholders in order to create collaborative partnerships and research opportunities; including networking, drafting proposals and contracts, and managing financial responsibilities.     • Coordinates and oversees networking and continuing education opportunities for professionals including PCH and community medical providers, medical students, researchers, educators, community stakeholders, and policy makers
  5. Performs miscellaneous job related duties as requested.
