Posting Note: Phoenix Children’s Hospital is seeking an Advanced Practice Practitioner (APP) to join our growing heart transplant team. This position will be one of four APPs providing both inpatient and outpatient coverage to the pediatric heart transplant program. In this position, you will be working alongside a close group of two heart transplant cardiologists, APPs, one nurse coordinator, and two medical assistants. You will also collaborate closely with a dedicated nutritionist, pharmacist, social worker as well as others on the healthcare team in the care of these patients. You will also work closely alongside and in direct communication with patients and families daily. This busy program follows approximately 150 heart failure/transplant patients and performs between 12-16 transplants per year. The pediatric heart transplant program is a part of the Phoenix Children's Hospital Transplant Center which also includes programs in bone marrow, kidney and liver transplantation. Required Qualifications: Pediatric Nurse Practitioner (Primary Care or Dual Certification) or Physician Assistant licensure Certification in area of educational training BLS/PALS Previous inpatient clinical experience is preferred but not required, specifically in an environment providing care to transplant patients or congenital heart disease General responsibilities include: Inpatient work: The team of four APPs provide inpatient and outpatient coverage to heart transplant patients Monday through Friday. This position will work rotating 8-hour days in conjunction with another part-time APP as a job share. Includes H & Ps, consults, and rounding on patients (primarily in the Cardiovascular Intensive Care Unit “CVICU”) for pre transplant, transplant and post-transplant patients Discharge education and coordination of care is extensive in this role Requires management of medically complex pediatric patients with advanced heart failure, mechanical circulatory support, as well as post-transplant care including immunosuppressant management You will be working alongside attending physicians as well as many other disciplines within transplant so good communication and interpersonal skills are required This position partners with three other (APPs) to provide 24/7/365 call coverage to the heart transplant program for organ offers and transplant Outpatient work: Clinic is located in the Heart Center at Phoenix Children's Hospital on the campus at 1919 E. Thomas Must be able to work autonomously and in partnership with multiple disciplines constantly Management of the transplant population including evaluations and consenting for transplant, as well as follow up and coordination of ongoing care Outpatient management of advanced heart failure and patients with Mechanical Circulatory Support Patient and family education is extensive in this position Supervising and working directly with dedicated transplant Medical Assistants and Registered Nurses Active coordination and participation in committee meetings related to transplant patient management

This position works in collaboration with the attending physicians and works within their scope of practice to provide excellent clinical care for patients. Plays a critical role in the patient family medical team. Professional who may practice in hospital’s inpatient and/or outpatient setting.

1. Conducts comprehensive health history, physical assessments and plans for infants, children and young adults with acute and chronic health conditions. Utilizes advanced assessment skills to manage a cohort of patients.
2. Orders and prescribes pharmacologic and non-pharmacologic interventions, and performs other therapeutic or corrective measures to treat illness and improve health status.
3. Formulates family centered plan of care. Performs daily documents as necessary. Collaborated and coordinated care with other team members.
4. Collects data and analyzes when necessary.
5. Performs any needed advanced procedures that are associated with their credentialing process DOP.
6. Utilizes the most up to date and evidence based practice for plan of care and able to alter the plan according to family needs and effectiveness or plan.
7. Serves as an educational resource to patient/family/ nursing staff/ provider team and other health care teams as needed.
8. Participates in own professional development by maintaining required competences, certifying body renewals, seeking educational offerings when available, and participates in the professional development of others which may include Advanced Practice Provider students, Residents and Medical Students. Acts as a preceptor/mentor for those new in department.
9. Demonstrates commitment to patient safety by bring up concerns in appropriate manner, reporting through appropriate channels, and ensuring change of practice when necessary. Demonstrates shared accountability of safe practice.
10. Performs miscellaneous job related duties as requested.
