Position Summary

Position Duties

  1. The CTC facilitates and coordinates the evaluation and OPTN/UNOS listing of potential transplant candidates, ensuring compliance with CMS and OPTN requirements.
  2. The CTC develops, monitors, and coordinates comprehensive care plans for the transplant candidate awaiting transplant in compliance with CMS, OPTN and transplant center requirements, ensuring required documentation is completed
  3. The CTC participates in continuous transplant call coverage for the transplant program and facilitates the transplant process when a donor organ becomes available for a PCH transplant candidate in compliance with OPTN/UNOS regulations and allocation policies.
  4. The CTC develops and implements comprehensive care plans for clinical care of transplant recipients under protocol including medication management; assessment and treatment of complications; laboratory management; interventions and discharge planning in both the inpatient and outpatient setting.
  5. The CTC communicates clinical assessments, findings and plan of care to members of the transplant multidisciplinary care team and the transplant candidate, recipient and family. This includes verbal and written communication to both internal and external audiences.
  6. The CTC participates in multidisciplinary transplant meetings, including, but not limited to patient selection committee and daily rounds. In addition, coordinates and participates in transplant candidate, recipient and family education.
  7. The CTC serves a clinical role model, patient advocate, and resource to staff, patients, and families. The CTC also participates in transplant quality meetings, performance improvement projects ,data collection and entry.
  8. The CTC participates in ongoing professional education in transplantation annually. Maintains involvement in professional organizations and reviews and maintains currency of transplant related professional literature. This includes ability to obtain the American Board of Transplant Coordinator Certification within the first 2 years of holding the position.
  9. Performs miscellaneous job related duties as requested.
