Position Summary

Posting Note: Fetal Care Network Nurse Coordinator - PCH is looking for an experienced RN, who has 5 years experience in the in NICU or L&D to work with the families of babies in utero, who have been diagnosed with a child who has special needs. The incumbent will work with sensitive information, provide emotional support and resources for the families during the pregnancy, and once the baby is born. Incumbent will be working with the families from the time the initial diagnosis is made, to the time baby is born. If you have this dynamic skill set of working with families before the baby is born; has the ability to provide education to the community about what PCH offers to help families with those special needs, then we want to talk with you! This is a Monday - Friday position, 8 AM - 5 PM, but work hours can be flexible. BSN is required.

This position is responsible for patient care services, workflow, and communicating activities for/with nursing and support staff of assigned program. This position requires guidance and support to ensure consistent provision of developmentally appropriate quality patient care in accordance with all applicable federal, regulatory, and professional standards and requirements. Provides a vital role in patient care at the Arizona Fetal Care Network at Phoenix Children’s Hospital.

Position Duties

  1. Communication Liaison
    • Acts as a communication liaison between patient, family, and interdisciplinary team by providing teaching and support to recognize limitations and strengths within the patient/family system.
  2. Identify patient and community learning needs
    • Assists with identification of patient and community learning needs from a group and individual perspective.
  3. Care Coordination and Continuity of Care
    • Collaborates with medical staff, nursing and auxiliary staff within the institutions and with outside providers for purposes of care planning. Cultivates and maintains effective interaction/ communication between team members and families to drive the care coordination process to facilitate continuity of patient care
  4. Clinic visit preparation
    • Works with the team to streamline clinic visits by performing prior authorizations and assuring families are prepared for clinic visits
  5. Data Collection
    • Maintains appropriate records to facilitate accurate data collection, patient tracking and timely preparation for the upcoming patient visits.
  6. Developmentally appropriate care
    • Provides developmentally appropriate clinical care within professional domain to ensure delivery of quality and developmentally appropriate patient care.
  7. Maintains up to date knowledge
    • Regularly reviews research-based literature and attends role related conferences to improve clinical care for specific clinical populations as well as compliance with regulatory bodies
  8. Clinical role model
    • Serves as a clinical role model, patient advocate, and resource to staff, patients, and families. Shares knowledge and skills of specialty area to assist in problem solving collaborations. Maintains skill, knowledge proficiency, and competency within specialty area
  9. Collaboration with program leadership
    • Works collaboratively with program leadership and lead coordinator to take an active role in planning, decision-making, and communication to healthcare team of issues affecting the interdisciplinary team and integration of patient care activities.
  10. Community outreach
    • Provides outreach to the community and servicing providers in order to educate the community regarding the services that can be offered to their patients, maintain relationships with referring provider, improve communication amongst the interdisciplinary team
  11. Performs miscellaneous job related duties as requested.
