Position Summary

Position Duties

  1. Assesses and identifies nutritional needs of patients and develops a medical nutrition therapy plan of care to improve medical and health status, with the patient’s psychosocial, ethnic, cultural and religious needs in mind.     • Coordinates and performs follow-up assessments and therapy to evaluate patient’s responses to care plan.
    • Documents nutrition care plans and recommendations accurately, concisely and adheres to documentation standards to ensure timely availability for coordination with the patient's interdisciplinary care plan.
    • Collaborates with multi-disciplinary health care team (including physicians, nurses, diabetes educators, and psychologists) to establish goals for patient's nutrition care plan that coordinates with interdisciplinary treatment objectives. Takes the initiative to approach the health care team with patient issues that may not be nutrition care related but significant to the patient's total care plan.
  2. Develops, conducts and evaluates educational materials and in-service programs that meet the individualized developmental, social, economic, cultural and learning needs of patients, families, health care team members, community members, and all interested parties
  3. Participates in hospital and department operational and quality improvement initiatives; proactively evaluates systems, practices and assists to create and implement changes to increase and enhance the effectiveness of the operations.
  4. Maintains professional and technical competency through regular participation in educational/training programs, in-services and by reviewing relevant professional publications and materials. Acts as a resource for families, community, and health care staff regarding nutrition issues.
  5. Performs miscellaneous job related duties as requested.
