Position Summary

This position is responsible for leading the diabetes education program within the Division of Endocrinology and Diabetes. This position oversees the building, implementation and maintenance of a self-sustaining diabetes education program both internally and externally, within PCMG and throughout the community This position receives supervision of work activities from a Manager or Director. This position supervises nursing and other professional support staff.

Position Duties

  1. Diabetes Program Management
    • Collaborates with the Diabetes personnel and participates in Diabetes events for the community and the medical center.
    • Coordinates ancillary services to ensure a comprehensive multidisciplinary approach to Diabetes care.
    • Coordinates and/or participates in multidisciplinary patient/family conferences and discharge planning with the case manager, physician staff and care team.
    • Develops and assists in the instruction of Diabetes educational programs for nurses, physicians.
    • Develops and recommends to division management ideas and information which will enhance, improve and update the Diabetes program.
    • Maintains ADA accreditation on a yearly basis to maintain ADA status. Keeps quality informed of all quality activities related to the Diabetes program.
  2. Diabetes Education Supervision
    • Oversees scheduling and staffing responsibilities for a team of diabetes educators.
    • Coordinates training and education sessions for patients/families, staff and industry professionals.
    • Acts as a liaison between the diabetes education team and various departments within the hospital, to increase exposure and advancement for the program.
    • Oversees and supports Endocrine volunteers to assist with special projects.
    • Provides administrative support for the diabetes education program.
    • Evaluates and modifies current standards of care, education materials and practice documents on a regular basis.
    • Collaborates with the CARE Program/Program Coordinator to bridge efforts and services between the two programs.
    • Provides diabetes education to patients and families, when applicable.
  3. Performs miscellaneous job related duties as requested.
