Position Summary

This position oversees the daily activities of the medication use process and provides comprehensive Pharmacy services to patients from neonate to young adulthood while performing the duties of a registered pharmacist as defined by the Arizona State Board of Pharmacy and Phoenix Children’s Hospital. This position also ensures compliance with Federal, State and local laws and regulations as well as proper storage and documentation. This position reports directly to the Manager, Pharmacy Operations and under the supervision of the Manager, Clinical Pharmacy Services and the Chief of Pharmacy.

Position Duties

  1. Accurately processes outpatient prescriptions and provides oral and written counseling information to the patient and/or caregiver when necessary. Provides oral and written information, monitors patients for optimal therapeutic outcomes; verifies appropriateness of medication selection for patient’s diagnosis, weight/body surface areas, age, allergies and drug interactions.
  2. Actively participates in the education and training of new pharmacy employees, pharmacy students, and pharmacy interns. Oversees technical and support staff to promote efficient daily workflow.
  3. Acts as liaison between the pharmacy and the medical and nursing staff for issues pertaining to pharmacy services. Participates in hospital and department operational and quality improvement initiatives; proactively evaluates systems, practices and assists create and implement changes to increase and enhance the effectiveness of the operations. Serves in a professional capacity on unit-based clinical teams providing information and suggestions for alternative therapies.
  4. Effectively communicates problems, unique patient needs and other necessary information to on-coming staff for enhanced continuity of care. Responds to all modes of communication within the department and hospital and proactively solves problems. Answers phones promptly and politely demonstrating PCH Values of excellence in clinical care, service and communication at all times
  5. Completes all assigned clinic and nursing station QA check assignments including follow-up and resolution of identified problems. Provides direct oversight and timely and accurate verification of final product for all aseptic parenteral and non- parenteral medication preparation, dispensing and distribution in compliance with state, federal statues and hospital procedures.
  6. Completes the annual aseptic technique certification by skills validation with a qualified observer and other annual competencies by skills lab and/or examination as required. Participates in a variety of professional development programs to ensure ongoing awareness and understanding of most current methodologies, practices and philosophies as applied to the relevant patient population and research area. Actively pursues self-education and ensures satisfactory completion of all State Board continuing education credit currency.
  7. Processes physician orders in the identified pharmacy information systems in an effective, efficient, timely, safe and accurate manner, and manages the computerized patient profile. Maintains appropriate competency and skills with automated dispensing and other related medication system technology used within the department and hospital to provide safe care.
  8. Initiates, processes and completes all associated incident, adverse drug reaction and controlled substance documentation reports and records to ensure ongoing compliance with regulatory requirements and standards. Prepares various other reports as necessary. Participates in data collection and reporting of Drug Use Evaluation, Quality Assurance and Performance Improvement activities. Maintains investigational drug study records and documentation as required by the study sponsor and accrediting bodies.
  9. Reviews and interprets physician orders for completeness and appropriateness with respect to the patient’s clinical status, age and/or other significant factors effective the safety and efficacy of the drug therapy prescribed. Ensures doses are filled and delivered within established departmental guidelines in a timely manner. Evaluates need for all non-formulary medication required. Verifies and ensures appropriateness of medication selected and recommends initiation and substitution of drug therapy when necessary. Suggests equally efficacious and cost effective therapy when appropriate. Works closely with all pharmacy personnel to assure safe and efficient delivery of patient care. Identifies problems and intervenes when appropriate involving drug therapies and documents interventions in identified data base.
  10. Performs miscellaneous job related duties as requested.
