Position Summary

Position Duties

  1. As primary advanced nursing care provider, delegates activities, as appropriate; provides timely feedback and review of care delivery to ensure maintenance of patient safety, infection control procedures, therapeutic environment, and team members' performance level, maintaining confidentiality as appropriate.
  2. Complies with all regulatory requirements (CMS and Joint Commission, UNOS, and Donor Network of AZ) in the care of the transplant patient during all phases of care - including pre-transplant, transplant, and post-transplant.
  3. Conducts comprehensive or episodic health assessments of specific diagnoses in both the inpatient and outpatient settings. Assessments include medical status, psychosocial status, cognitive status, personal assets, and limitations to learning.
  4. Continually evaluates patient's response to care, teaching and discharge plan. Documents patient progress in appropriate charts and shares observations and assessment outcomes with health care team and family to ensure timely communication and understanding of patient health status, progress, and ongoing needs.
  5. Establishes systems and processes to promote and improve operational efficiencies for the transplant program.
  6. Responsible for the coordination of the transplant candidate evaluation including placing orders, collecting and communicating findings to to transplant physician partner(s), educating patient and family, communicating with transplant financial services, as well as compiling and presenting findings at patient selection conference.
  7. Maintains professional and technical competency through regular participation in a variety of educational programs and in-services, reviewing relevant professional publications and materials, and participating in professional societies to ensure ongoing understanding of most current methodologies, practices and philosophies as applied to patient care population. Participates in staff and multi-disciplinary care team education.
  8. Participates as an active member of the Solid Organ Transplant Multi-disciplinary team. Participates in patient selection meetings, patient rounding, family conferences regarding transplant patients and quality and process improvement projects. Participates in transplant donor, recipient and patient/family call.
  9. Works collaboratively with patient care team and family members to establish and implement plan of care to ensure coordination of activities and ongoing communication of patient health status. Has primary role in planning and coordination of discharge, including patient/family education.
  10. Performs miscellaneous job related duties as requested.
